Parents or guardians may complete this Physical Education Opt Out Form to request their student be excused from P.E. class. This form must be completed each semester the opt out is desired. Physical education grades for students who have opted out of P.E. will be listed as excused on the report card and transcript.

If your student is going to be absent from school, the attendance office must be notified by using the Absent Request Portal located in your Infinite Campus Parent Portal. This is the preferred method for reporting a student’s absence.

After the student has completed Driver Education, begin in the High School front office where we have the DOT forms. Email or bring the student’s current permit to the office & we will start the form for you along with a letter. Do this before going to the DOT.
